STEP 1
Complete the Online Form
👉 Fill out the official SLC Enrollment Form and attach the required documents, including the PSA Birth Certificate.
STEP 2
Settle the Downpayment
👉 Use the secured payment link or static QR code provided in the online form. Pay using any e-wallet or online bank transfer, take a screenshot of the proof of payment, and upload it within the form to finalize the enrollment process.
STEP 3
Submit Physical Copies
👉 Drop by the Registrar's Office at your convenience within the required submission dates to hand in the original copies of the Report Card (SF9) and Certificate of Good Moral Character.
